Now I'm curious, does adding one more day every 4 years work out perfectly or, will they at some point add 2 days or, skip adding the extra day one time?

It works very well, however, every once in a while a leap second is needed.

Leap seconds are added to account for variances in the Earth's rotation not problems with the leap year rule.

BTW, the complete leap year rule is:
Year divisible by 4 - yes
Unless divisible by 100, then no
Unless divisible by 400 then yes
