I would cut the doctor a little slack. He could be fresh out of med school and is having some doubts. One day he's bragging to some gal that he's going to be a doctor, the next, he's knuckles deep in some guys ass trying to make small talk. "So, do you like fishing? "When I grab your balls, cough".
 
“Misspell” the amount on the check.
Just claim it’s dyslexia when they catch you. That should buy you enough time to leave the country for good. That’s the only way out of this.
 
My bet is that he dictates the information from your visit into a medical content management machine and never checks to see if the AI spelled the words correctly. Kind of like using the dictation option on your phone message app.
